My Future Care - Introduction to Advance Care Planning Workshop

We all want to have a say in what happens in the last part of our lives. This free workshop delivered by Palliative Care WA will help you plan ahead and let your families and health care providers know what you want.
By making plans that cover your future care, lifestyle, health and finances, you’re not only working out what you want, you are also making things easier on those around you. This process is called Advance Care Planning.
In this FREE workshop we will cover information on:
- What’s involved in Advance Care Planning
- How to get started
- How to talk about your wishes for the last stage of your life with your family, loved ones and health care providers.
The workshop is suitable for all members of the community. You can be any age or have any level of health.
We welcome and encourage the attendance of people with disabilities, and will endeavour to accommodate any access needs disclosed. The venue has accessible parking bays and accessible toilets available. Please contact Ruth on 9257 9958 to discuss any further access or dietary needs.
